Procedure to Fill Application Form
There are two procedures to fill the application form :-
1. Online application:
Online application: Fees of online application form is Rs 1200 (General/OBC/ SBC) and Rs 600 (SC/ST/PH>=40%). The fees can be paid in two ways.Payment through challan:
Fillup the online application form, print on A4 size paper, deposit the challan in ICICI Bank along with applicable fee printed on the challan. Attach one copy of chalan, self attested copy of domicile certificate, X and XII certificates and reservation eligibility certificate as the case is. Send to coordinator through registered post.Payment through DD:
Prepare DD of Rs 1200 (General/OBC/SBC) and Rs 600 (SC/ST/PH>=40%) payable to Coordinator, JET/Pre-PG, MPUAT at Udaipur. Fill the online application form. Attach DD, self attested copy of domicile certificate, X and XII certificates and reservation eligibility certificate as the case is. Send to coordinator through registered post.2. Offline application:
Download the blank application form , print on A4 size paper (Photo copy also allowed).Prepare DD of Rs 1400 (General/OBC/SBC) or Rs 800 (SC/ST/PH>=40%) payable to Coordinator, JET/Pre-PG, MPUAT at Udaipur. Fill the application form. Attach DD, self attested copy of domicile certificate, X and XII certificates and reservation eligibility certificate as the case is. Send to coordinator through registered post.
3. Printed application form:
Application can also be purchased from Coordinator office on cash payment of Rs 1400 (General/OBC/SBC) and Rs 800 (SC/ST/PH>=40%). Candidate desirous to get application form through post, submit a DD of Rs 1500 (General/OBC/SBC) and Rs 900 (SC/ST/PH>=40%) payable to Coordinator, JET/Pre-PG, MPUAT at Udaipur. Fill the application form. enclosed self attested copy of domicile certificate, X and XII certificates and reservation eligibility certificate as the case is. Send to coordinator through registered post.In all the cases submission of hard copy in JET office is essential. Application should reach to the coordinator before last date. For postal delay this office will not responsible. Applications received after last date will not accepted by this office. The deposited fees is non refundable. Mentions “Application Form” on envelop.
